To make a life certificate, the applicant person has to attach the necessary documents related to Aadhar card, passport size photo, pension with the application form. After which one has to be present personally at the authorized pension disbursing agency.

Steps to Download Uttar Pradesh Life Certificate Online:

1. First of all you have to visit the official website of Jeevan Pramaan. https://jeevanpramaan.gov.in/#home
2. On the home page, you have to click on the option “Get a Certificate”. On clicking, a new page will open.
3. Here you need to download the PC/Mobile/Tablet application to register for Jeevan Pramaan.
4. After that you have to open the downloaded app. And then you need to fill the following information.
   1. Shanbhogi’s name
   2. aadhar card number
   3. mobile number
   4. PPO Number
   5. bank account information
   6. Name of Pension Sanctioning Authority
   7. pension account number
5. After filling all the details correctly, your Aadhar card will be verified. After this, the option to download the certificate in PDF format will open in front of you.
6. You have to click on this option. After this you will get the UP Jeevan Pramaan Patra download.
